Forms for importing livestock into Western Australia

Page last updated: Thursday, 31 May 2018 - 4:02pm

Western Australia is free of many diseases that are endemic to other areas in Australia. A livestock importer is required to certify the disease status of livestock being imported into WA and pre- and post-arrival testing and treatment protocols are often required.

The LB1 Health certificate for movement of stock to Western Australia is used to provide this information. Exemption forms referred to in the LB1 can also be accessed below.

A PDF viewer is required to open the forms below. If you do not already have a PDF viewer installed on your computer, you can download the latest free version of Adobe Acrobat reader.

Note that while you may fill in the forms electronically, forms must be printed out for signing.
